Measurement Specification Form
This form needs to be completed and emailed back to us within 3 business days after receiving your quote. Your quote will be considered invalid after 3 days.

A - Only fill out what applies to your order.
B - Make sure measurements are taken according to the measurement guide attached to this file.
C - Only use the color charts we provide.
D - We urge you to take your time and read through our FAQ / Terms & Agreement before ordering to fully understand what to expect and the commission process.

Guidelines & Important Links
1 - Please follow the instructions given on the attached Measurement Guide for accurate measurements.
2 - Please take in consideration of any possible personal weight gain/weight loss during the commission process.
3 - Make sure all specifications and requests are carefully listed below. CarrotAnime is only responsible and obligated to make sure these specifications are done on the costume and shown in the pictures provided when costume is finished.
4 - Please include all reference images and related images in a folder that's zipped (.zip or .rar format). If there are multiple folders, please label them accordingly.
5 - Only use the color chart provided by CarrotAnime, do not use a third party color chart.
6 - Take into consideration possible color differences due to different monitors and resolutions.
7 - Please take your time to carefully read all of our Guides, Policies and Agreements, to know what to expect and to fully understand the custom commission process.
8 - We do not supply progression pictures. Pictures of the finished costume will be provided ONLY for costume/cosplay goods that are not in our store.
